    The state of Montana allows Direct Access to physical therapy services or “self referral” to practitioners like Momentum Physical Therapy. This allows patients to be evaluated and treated by Dr. Pakula without needing a PT “prescription” from their physician. Direct Access saves you time and money by expediting your treatment, relief, and recovery.

    DO YOU BILL AUTO OR WORKER'S COMP INSURANCE?

    Yes, we bill most auto insurance and worker's compensation insurance. We are out-of-network with all other insurance companies. We provide a superbill that you can submit to your insurance company for out-of-network reimbursement.
